Appendicitis is an inflammation of the appendix which is a tubular organ present at the beginning of the large intestine. It extends from 2 to 3 cm below the ileo-cecal junction and is usually 8 to 10 cm long. Appendicitis leads to pain that begins in the central abdomen and later localizes in the right lower abdomen. Though it can occur in any age, it is usually seen in younger age group of 10 to 30 years. Left untreated, it can lead to perforation of the appendix or an abscess formation.
Causes of appendicitis
A blockage in the lumen of the appendix due to fecoliths can lead to inflammation. Lumen of the appendix may also get obstructed due to the inflammation of the lymphoid tissues.
Signs and symptoms of appendicitis
- Appendicitis usually starts as a diffuse pain in the central abdomen which later localizes to the right lower abdomen. Sometimes appendicitis may present with atypical locations of pain too.
- Tenderness on touch in the right lower abdomen.
- Nausea and vomiting
- Lack of appetite
- Diarrhea or constipation
Appendicitis is usually a clinical diagnosis. However, the following investigations are helpful to rule out any differential diagnosis:
- Complete blood count may reveal mild increase in the white cell count. Significant increase in the white cell count may be suggestive of a perforated appendix.
- Urine analysis
- Imaging studies like USG of the abdomen or a CT scan of the abdomen as per the clinician’s prerogative.
Treatment of appendicitis
Surgical removal of the appendix is the standard of care for treatment of appendicitis. This can be done laparoscopically or even with single incision surgery through the navel. Open surgery may be needed rarely for very complicated cases. Laparoscopic surgery leads to much lesser pain and early recovery. Most patients are able to get discharged from the hospital the next day after their surgery. In some cases, doctors may choose to treat with antibiotics initially and perform the surgery after a few weeks.
